With over 18years of working in various healthcare settings; in patient’s homes, GLEs, inpatient psychiatry and medical, community settings as a visiting nurse, long-term care, I have an extensive experience in assessment, care coordination, case management, psychopharmacology and traumainformed care. As a DNP prepared provider who is specialized in psychological trauma, dedicated to screening for traumatic experiences and other social determinants of health which can have an impact on the patient’s physical and mental wellbeing. As a healthcare provider who is certified in both psychiatry and psychiatric mental health, my goal is to offer compassionate, comprehensive care that is not only thorough and covers all aspects of a patient's needs, but also delivered with empathy, understanding, and genuine concern for my patient’s well-being; by providing excellent care while actively considering the patient's emotional and personal situation alongside their medical needs. Providing holistic and personcentered care remains the core values and goals of my practice.
I currently treat patients with acute and chronic psychiatric illnesses, substance use disorders, alcohol use
disorder, and other neurological disorders such as epilepsy, learning disabilities, autism, ADD and ADHD. Trauma-informed care is the focus of my practice. With certification in Family medicine, I am able to rule out medical conditions that may mimic psychiatric illness.
In my earlier roles, I have worked in Long-term care facilities, acute care hospitals as an RN in both medical and psychiatric mental health settings, group living environments, and community health programs (both as a visiting nurse and a community nurse providing case management to individuals in their homes), and as an NP in both in community and inpatient. I am therefore well equipped with the skills and knowledge required to provide exceptional care to both patients and families/caregivers I various settings
I believe in providing clear communication and educating my patients about their health as an intervention that empowers them to make informed decisions regarding their health. I recognize the fact that a patient’s family or social support network play an integral role in their overall wellness and health, and therefore ensure that with the patient’s permission, this need is met. I therefore focus on providing individualized care based on each individual’s needs.
Initial assessments are focused on understanding the person as a whole. All assessments are performed in a setting that is relaxed and strives to protect the individual’s privacy and health information. Follow-up appointments are scheduled based on the needs of each individual.
